APPLICATION SUBMISSION INSTRUCTIONS:
1. Create an account or login to your existing ISAAC account.
2. Complete ALL fields of the Hockey Equipment Request.
3. Click Add to Cart at the bottom of the page when you have completed ALL application fields on the form. (NOTE: You will be prompted to complete any incomplete fields at this time.)
5. Once all ALL fields and required uploads have been completed, click Checkout.
6. Your application will now appear in the Shopping Cart with a $0 balance owing. Click Process Order to submit the Hockey Equipment Request.
PLEASE NOTE: if you do not receive a confirmation email from The ISAAC Foundation (firstname.lastname@example.org), your application WAS NOT successfully submitted through our online application process. Please contact Olivia at (509) 325-1515 for assistance.
